چکیده

BACKGROUND The purpose of this study was to compare complications and dental implant success between the bone-impacted fibula free flap (BIFFF) and the traditional fibular free flap used in mandibular and midface reconstruction. METHODS Retrospective review of all patients undergoing BIFFF or traditional fibular free flap reconstruction from 2001 to 2009 was undertaken. Complications related to the BIFFF and traditional fibular free flap site of reconstruction were compared. Dental implant success rates for each type of flap were compared at 1-year intervals for 5 years. RESULTS One hundred fourteen patients underwent 81 BIFFFs and 35 traditional fibular free flaps. No significant difference in complications between BIFFF (20.9%) and traditional fibular free flap (25.7%) reconstruction was observed. Logistic regression analysis revealed only the site as a predictor of both single and multiple complications. At 5 years postimplantation, dental implant success rates were 95.5% and 77.1% for BIFFF and traditional fibular free flap, respectively (p = .006). CONCLUSION BIFFF reconstruction is a novel surgical technique that may improve long-term dental implant success rates with no additional risk of complications. © 2015 Wiley Periodicals, Inc. Bone impacted fibular free flap: a novel technique to increase bone density for dental implantation in osseous reconstruction.

BACKGROUND Fibular free flap (FFF) bone has thick cortical bone surrounding a fatty marrow. The cortex has sufficient density for dental implantation, but the marrow limits bone stock. A novel technique was devised to increase bone density: the bone-impacted fibular free flap (BIFFF). The purpose of this study was to: (1) describe the BIFFF technique; (2) evaluate the bone density of BIFFF; and...

MICROSURGICAL TREATMENT OF LONG BONE DEFECTS USING FREE VASCULARIZED FIBULAR TRANSFER

Management of long bone defects occurring in accidents or war injuries is a difficult problem, From 1978 to 1986, 11 patients were treated for long bone defects of the humerus ( 10 cases) and radius (one case), using free vascularized fibular transfer by microsurgical technique .The patients were followed for 4 months and in all of them, selective angiography and scanning revealed exellent...

Repair of Minor Tissue Defect in Hand by Transfer of Free Tissue Flap from the Toe

Background:   To introduce our experience of using the free neurovascular flap from great and second toe. Methods:   Thirteen patients (fifteen fingers) sought surgical treatment for soft tissue defects of the hand at our medical institutin between March 2006 and September 2009. In two patients, fibular side skin-nail flaps of great toe were applied to cover the dorsal defect of distal thumb. I...

New modified free chimeric fibular flap design for head and neck reconstruction.

BACKGROUND The free fibular osteocutaneous flap is a commonly used flap for mandible and buccal mucosa reconstruction in head and neck cancer surgery. However, the skin paddle restriction from the intermuscular septum often limits the soft tissue reconstruction. METHODS We have proposed a new modified chimeric fibular osteocutaneous flap design based on the combination of a traditional fibula...

Reconstruction of the pediatric maxilla and mandible.

BACKGROUND The creation of osseous defects in the upper and lower jaws in children is an uncommon occurrence. It is therefore likely that a head and neck reconstructive surgeon will accumulate only limited experience in restoring such defects. We have reviewed 7 pediatric bone-containing microvascular free flap reconstructions in 6 patients for reconstruction of the upper or lower jaws. Three p...
